We are seeking a Clinical Practice Specialist. In this position, you'll serve as the Telehealth Nurse Case Manager, using cutting-edge technology to monitor patients and educate them on maintaining and improving their health. You will also be the Infection Control Specialist for teams across our organization. You will demonstrate advanced knowledge of best practices and regulatory requirements, providing feedback and recommendations to our leadership team and staff. This person will ensure that the agency's Infection Control policies and practices are current, meet regulatory guidelines, and are practiced consistently.

Job Duties & Responsibilities
  • Identify and provide targeted clinical education to promote best practices, mitigate risks, improve outcomes, and assist with clinical record review as needed
  • Facilitates/participates in performance improvement
  • Ensures survey readiness and compliance with CLIA regulation
  • Acts as Education Coordinator for the organization and assists with performance improvement for the network
  • Demonstrates knowledge of Home Health/Hospice clinical, regulatory, PI, and Educational Requirements
  • Complete reports for board, assist in PI projects, and provide staff educational programs
  • Responsible for the monitoring, interpreting, and appropriate intervention of biometric data that is remotely transmitted by Home Health patients
  • Provides patient education for self-care management of chronic diseases
  • Provides resource assistance and education to Home Health Staff in chronic disease management

Skills & Requirements
  • RN with current Arkansas license, BSN preferred
  • Five years of experience in nursing, at least three years in a Home Health or Hospice setting
  • Experience in infection prevention/control, cardio-pulmonary, teaching, and consultation preferred
  • Experience with performance improvement and data analysis preferred
  • Experience with chronic disease management required
